MERISERA® Anti-D (IgG+IgM) is a blend of monoclonal IgM and IgG anti-D antibodies for comprehensive Rh(D) blood group typing. The IgM component provides immediate spin/slide agglutination for routine Rh typing, while the IgG component enables detection of weak D (Du) variants via the indirect antiglobulin test (IAT/Coombs test). This dual-antibody format is the blood bank gold standard for complete Rh(D) determination.
Why IgG+IgM Matters
Standard IgM-only Anti-D detects most Rh(D) positive samples but can miss weak D (Du) variants — individuals with reduced D antigen expression. If mistyped as Rh-negative, these patients may receive Rh-negative blood unnecessarily (wasting a scarce resource) or, if donors, their Rh-positive units may be transfused to Rh-negative recipients causing alloimmunization. The IgG+IgM blend ensures no weak D is missed — IgG antibodies sensitize weak D cells for detection by AHG in the Coombs phase.
